To specify the default HTML topic file for a compiled help file 
Open a project (.hhp) file, and then click Change Project Options.  
Change Project Options Specifies files, compiler, window, and other project settings. Click the General tab, and then in the Default file box, enter the name of the HTML file you want to appear when a user opens the compiled help file.

我试过了，没有问题，只要在property中设置一个default html就没问题了。
你也可以decompile 一个现有的help文件，参考一下。
