When I run a form with a menu attached, I get an FRM-10221             followed by a FRM-41810.

Solution:   This sequence of errors simply means that Forms could not locate your MMX file. 
 You should check the following things:   
      * Make sure the MMX file exists.       
      * If you are running Oracle Forms make sure the directory in which your MMX file resides in the the appropriate FORMSxx_PATH variable:                FORMS45_PATH, FORMS50_PATH OR FORMS60_PATH depending on the version of forms you are running.          
    * For Windows 32 bit operating system, check the "START IN" property for the short cut.

注册表中指定forms60_path为菜单所在路径。
在form级的属性中把菜单文件名填写完整。
